1. What forms do I need to fill out if I get MARRIED/DIVORCED, or NAME/ADDRESS CHANGE?

- Reimbursement Account
- Vision
These forms MUST be completed and turned into the Human Resource Department WITHIN 31 DAYS OF THE CHANGE IN STATUS. Please plan for approximately 15 minutes or longer to fill out these forms. These forms can be found at the Human Resource Department.
2. What forms do I need to fill out if I have a NEWBORN child, ADOPT a child, or OBTAIN CUSTODY of a child and want to add them to my insurance?
- Life Insurance
- Medical, must fill out health questionnaire for adoption and custody.

- Vision
- W-4

3. When can I CHANGE INSURANCES?
You can only change your insurance during Logan City's official open enrollment period (approximately May or June), unless you have a family status change as outlined above. These changes will be effective on July 1 of that year.
4. When can I DELETE/ADD INSURANCE?
You can only change your insurance if you have a family status change as outlined above or during the City's open enrollment period.
5. Where can I find a list of PREFERRED PROVIDERS?
- Preferred Providers can be found by calling the 1-800 numbers found in the Benefits section of this web site.
- Booklets are available in the Human Resource Department.
6. How long does it take to get my Medical/Prescription or Dental card?
It usually takes 4-6 weeks after the premium has been deducted from your payroll check.
7. How do I turn in receipts for DEPENDENT CHILD CARE EXPENSES and UNREIMBURSED MEDICAL EXPENSES?
- Expense Voucher (Human Resource Department)
- Obtain original receipt
- Make a copy for your files

- The check will be mailed to the employees home address.
8. How do I do DIRECT DEPOSITS?
- Go to Human Resources or Payroll and fill out Direct Deposit Form.
- Must have Voided Check, (Deposit Slips will not work).
- It usually takes two (2) pay periods to begin depositing your checks directly into your account.
9. When do VACATIONS, SICK LEAVE, and HOLIDAY LEAVE begin?

Sick Leave

11. How often can I CHANGE MY 401K CONTRIBUTIONS?
Every pay period.
